Is it realistic for a smaller business to compete in Los Angeles or San Francisco search?

Not on head terms, and we would rather say that plainly than take a retainer against it. National agencies with years of accumulated authority and substantial content investment already hold "SEO Los Angeles" and its equivalents, and entering that race means spending significantly to arrive third at best. What is entirely realistic is narrowing until you are competing in a race you can win: a specific vertical, a specific neighbourhood, a specific service specialisation, or some combination. What matters instead is topical authority in the subject area, content that competes with well-resourced national players, technical performance at scale, and increasingly AI citation presence, since technical buyers are early adopters of generative search for research. The competitive set is also completely different: you are not competing with other San Francisco companies but with everyone selling the same category anywhere, which means the content bar is set nationally. Diagnosing which side of this line a California business sits on is the first and most consequential decision in the engagement.

How significant is Spanish-language search in California?

Significant enough to be a genuine strategic channel rather than a consideration, and consistently under-served relative to its volume.
